If you are unable to make it to your appointment, please provide us with a minimum of 24 hours’ notice to cancel. Appointments not cancelled 24 hours in advance are subject to a $100 fee. This policy is standard in the medical and mental health fields and will be strictly enforced. If you have any questions, please contact the office.
Rates vary depending on your insurance or if you are self-pay. For specific rates, please call the office with your insurance information so we can assist you.
Payment is expected to be made at the time of the appointment. We accept cash, check, and all major credit cards.
We are in-network with Medicare. We are out of network with all other insurance plans including Medicare Advantage/Replacement plans. Services may be covered in full or in part by your health insurance plan. As the patient, it is your responsibility to carefully check coverage with your insurance company before scheduling an appointment. We do not accept any Medicaid or Medicaid Replacement Plans.
